It is just one of a catalogue of blunders revealing how ambulance services are cracking under the strain.

An investigation reveals there were 153 serious incidents over two years from 2014, where there was a critical delay in getting to a patient or getting them into a hospital.

The figure is up from 97 in the previous 24 months.
